Terms and conditions for Gift Certificate

All gift certificate purchases are final and non refundable. Please email chicagonorthwest@fit4mom.com to provide the name and email address of the intended recipient. If you do not want the gift certificate added to their account until a specific date (in the case of a surprise gift) please let us know.
